Use the schedule below to select and request an appointment. Your appointment is ONLY confirmed after you receive an e-mail confirmation to your appointment request. Appointments are confirmed during normal office hours. 

If you are a new patient please fill out the in-take form you will receive after your appointment is confirmed. Use your legal name in your request.

Do not use this for requesting same-day appointments.
