Use the schedule below to select and request an appointment. Your appointment is ONLY confirmed after you receive an e-mail confirmation to your appointment request. Appointments are usually confirmed during normal office hours. (Please note: The office will be closed Wednesday afternoon Oct. 16- Friday October 18, 2024. Appointment requests will be acknowledged on Oct. 20th).

If you are a new patient please fill out the in-take form you will receive after your appointment is confirmed. Use your legal name in your request.
