Schedule An Appointment

Use the schedule below to select and request an appointment. Your appointment is ONLY confirmed after you receive an e-mail confirmation to your appointment request. Appointments are usually confirmed during normal office hours. 

If you are a new patient please fill out the in-take form you will receive after your appointment is confirmed. If you are an existing patient, please use your legal name in your request.

Please do not use this for requesting same-day appointments.
(Of course you may always call the office for an appointment at (845) 357-2806).

PLEASE NOTE: The office will be closed Monday April 22-Wednesday April 24,  Monday-Tuesday April 29-30 and Monday-Tuesday May 6-7.
