Please print and fill out the new-patient registration form prior to your first visit. (Click here to print the Patient Registration.) Our hours are Monday: 9:00-12:00 and 4:00 to 5:00, Tuesday: 9:00 to 5:00,  Wednesday 9:00 to 6:45,  Thursday 9:00 to 4:00, and Friday 8:30 to 12:45. The office is on the second floor. An elevator is available and the office is handicap and wheelchair accessible.
